plz plz read..large pores at 17...help ASAP!!?

im 17f and some years back i hd some clogged pores on my cheeks(part of cheek which is below my eyes and close to the nose). at that time i did nt know how to treat them or get the clogged debris out of the pores,but after abt a year,i learnt to get rid of it, but the pores never closed and they are now large and visible. it's like i hav many on both my cheeks. they accumulate some really thin and small hair (1 hair each pore) which can only be seen under a magnifying mirror, and they also accumulate very flat white flakes which is really tough to get out,i use precision tweezer vry carefully to bring them out(done under my magnifying mirror) . i regularly cleanse my skin,tone and moisturise it with loreal pure zone (the blue bottle range,i use their exfoliant gel,pore minimising toner and moisturiser).also my skin is normal,and my eating habits are really healthy
plz tel me how to get back smooth surfaced skin and get rid of these open poreS
